We serve uninsured and underinsured adults and will never turn anyone away due to insurance status or inability to pay. Our clinic operates on a sliding scale fee system, with visits starting at $50.
What do you need to bring to your appointment?
A list of current medications
Any form of identification (driver’s license, school ID, work badge, etc.)
Relevant medical records (if available)
If applying for the Sliding Scale Program, please also bring:
Proof of household income (recent pay stub, tax return, or benefits letter)
Proof of address (utility bill, lease agreement, or official mail)
